2021 Player Forms

All new children to the GCYFL must provide an official stamped birth certificate or a valid passport by July 19th, 2021 to participate. No copies, faxes or scans will be accepted. Only originals. All proof-of-age documents will be returned after book certification on August 12th. 

 

2019 returning Ducks or GCYFL players from another chapter will not need to show a proof-of-age document again.

 

The GCYFL Physical form (#5) is due on the first day of practice and must be filled out, signed & stamped by your Doctor, Physician or a Nurse Practitioner. No child may practice or check out their equipment for the Fall season until we receive all documents & full payment.
